SUS was born in the midst of the country's redemption struggle, as a major expression of the ideas of Brazilian sanitary reform, a movement aimed at achieving a genuine social revolution based on the horizon of socialism that will be grafted by health reform.
In 1988, the civil constitution would proclaim health as the right of all and the duty of the state, the unprecedented and historic landmark of our formation as a state. A new era of hope and empowerment is emerging. The Constitution gave birth to what appeared to be a solid social protection system.

Although the Brazilian Health Reform Movement aims to create a state health system, as stated in the report of the 8th National Health Conference of 1986, the balance of power given at this stage allowed for the inclusion of private initiative within the SUS. which is bequeathed to us by a system that calls itself "unique" but is supplemented by a private enterprise.
It is quite true that the constitutional SUS was not ready from the beginning, but was created or "carved with ordinances," some would say. Funding for the Unified Health System, for example, remained relatively volatile until the entry into force of Constitutional Amendment 29 of 2000, which established minimum resources for financing public health activities and services in the three spheres of government. And so, this way sui generis, SUS is consolidating …
The fact is that the decentralization of this policy in the countries and municipalities has undoubtedly contributed over the years to significantly improving health outcomes through access to services and actions with a humanitarian orientation in health care, helping to combat inequalities.
There have been many disputes over SUS since today that can hamper its effective consolidation as a universal healthcare system. The very notion of universality is called into question when market sectors consider the proposal for "universal health coverage" in an attempt to fragment the right and continue the process of codification of health.
We are currently witnessing an open mouth and a sense of powerlessness about the brutal deprivation of social rights, hard won by the Brazilian people. Unfortunately, SUS and the universal right to health, together with a number of hard-won social rights, are endangered.
What we thought was solid seemed to be collapsing in the air. Even our young democracy. The past seems to want to sneak into the present, to chase us, into constant dispute. Even the importance of Democracy is controversial.
In times of "post-truth" and "fake news", it is urgent to affirm the fundamental value of our democracy and our Common Health System, born precisely in this same democratic context. Democracy and SUS were born in a relationship of internal dependency.
There are many challenges ahead that require us to be ready to fight and to stand up. By amending Constitution 95/2016, which freezes public health spending for 20 years by setting a ceiling on inflation spending, we are facing a specific opportunity to eliminate SUS.
Not only that, Union spending proposed by EU 93/2016 by 2023 foresees a deviation of up to 30% of government revenue for purposes other than the original, which caused, in 2017 alone, a loss of 113 billion from the social security budget, according to SIGA Brazil.
However, Economy Minister Paulo Guedes is not pleased, saying he intends to link all revenue from the Union, the states and municipalities, that is, 100% of the budget. This would mean the termination of constitutional obligations for minimal investment in health, education and well-being.
The proposal for social security reform, in addition to being a very serious attack on all Brazilians, seems to put an end to the costly notion of what we mean by constitutionally established social security, with the revenue allocated to each area: health, care and more. Social Security.
Not to mention the devastating unemployment that has led thousands of people to physical and mental illness, a grim scenario undoubtedly exacerbated by the 2017 Labor Reform. physicians and the harmful changes to the National Primary Care Policy (PNAB).
In this scenario of serious social failure, the private system offers additional forms and tends to expand, threatening the survival of universal and public SUS.
The challenges we face are enormous. However, they all require us to create broad and unrestricted social cohesion around the protection of public, universal, integrated, fair, quality and adequately funded SUS.
With the absolute conviction that understanding of the right to health as a universal social right must be a banner for the entire Brazilian society.
